Living Well with Chronic Conditions Workshop Announced, July 2018
Community Partners is sponsoring the workshop, Living Well with Chronic Conditions, which guides people in exploring healthy ways to live and maintain an active lifestyle while managing the symptoms of chronic conditions such as diabetes, arthritis, cancer, chronic pain, heart disease and anxiety or depression. It teaches practical skills to manage the depression and isolation that accompanies chronic conditions. Leave with tools and confidence that make a difference in how you live.
This interactive workshop is open to adults with chronic conditions, family members and caregivers. Trained leaders guide participants in finding practical ways to deal with their conditions. This evidence based workshop was developed by Stanford University and uses a standardized, proven methodology to help people develop goals for improved nutrition and exercise, a better understanding of treatment options and improved communications with family and doctors about their health plans.
